Nomination Form for Sports Hall of Fame

Athletes and coaches, both living and posthumous entries, may be nominated.

An Athlete Nominee must be out of school for a minimum of ten years and have participated for at least one year in a Cathedral Varsity Program. They should have excelled in their sport(s).

A Coach Nominee must be retired from coaching at Cathedral and must have made a significant contribution to Cathedral athletics.
